SERVICE DESCRIPTION

Diabetes Footcare

The Diabetes Footcare service will teach patients self-management strategies by focusing on improving patient knowledge regarding diabetes and footcare: understanding A1C and the importance of engaging in proper monitoring of glucose levels; highlighting the relationship with healthy eating and regular exercise; educating patients regarding the role and proper use of respective medications; monitoring patient’s clinical outcomes and referring the patient to appropriate services per CDA guidelines.

Goal:

To ensure early identification and treatment of any issues with patient’s feet, reduce morbidity and improve quality of life.

Target Population:

Patients newly diagnosed with DM and patients with poorly controlled DM

SMART Objectives:

•Increase access by early identification through screening and assessment                      

•Increase healthy lifestyle choices and

•Improve physical and mental health of diabetics identified as at risk due to poor disease management per CDA guidelines
