STOP provides education; self management and skills training to patients who smoke. The program offers screening, provides information and motivational counseling, access to pharmacological supports and organizes follow up appointments. Systematic, interdisciplinary collaboration between primary care professionals is integral for optimal care provision. Current guidelines for smoking cessation recommend the combination of pharmacological interventions and psychosocial measures such as counseling using CBT strategies and ongoing support. Key outcome of interventions is to reduce the risk of patients developing chronic diseases commonly linked to smoking such as heart attack, stroke and cancer.
