Our History
Prescott Family Health Team is a provider-led, non-profit organization that had originally started with a single physician’s practice in1968 (under the guidance of the late Dr. Gunvant Bhatt).
In July 2004, we received the Family Health Network (FHN) status followed by Family Health Organization (FHO).
In 2007, we received the Family Health Team (FHT) status and since then we have been serving our patients with a team of four physicians and over 14 staff across two clinic sites, Prescott and Brockville.

Mission
To improve the quality of life for our patient population by expanding and improving primary care resources available to the community, with a particular emphasis on patient-focused programs and services that address the most complex and prevalent patient needs.
Our Team
Prescott Family Health Team (PFHT) is a group of health care professionals working collaboratively to provide comprehensive and accessible health care within two clinics located in Prescott and Brockville. Services are provided by an intradisciplinary team of healthcare providers.
