DIABETES PROGRAM
PROGRAM DESCRIPTION
The Diabetes Program is provided in collaboration with our community partner Rideau Valley Diabetes Services (RVDS). The program will augment existing FHT services by providing patients with screening, early diagnosis, timely management, treatment and education to promote optimal outcomes. Over eleven million Canadians were estimated to have diabetes or pre-diabetes in 2019 with continued significant growth anticipated in the number over the next ten years. The Canadian Diabetes Association strongly believes that there is a clear and growing need for investments in strategies, policies and programs to prevent Type 2 Diabetes as well as to manage all types of diabetes in Canada. The risk target population includes patients per the CDA guidelines. The program will teach self-management strategies by focusing on improving patient knowledge regarding diabetes: understanding A1C and the importance of engaging in proper monitoring of glucose levels; highlighting the relationship with healthy eating and regular exercise; educating patients regarding the role and proper use of respective medications; monitoring patients clinical outcomes and referring patient to appropriate services per CDA guidelines. The program is designed to further health deterioration; preventing or delaying the progression of Coronary Artery Disease; HTN, Peripheral Neuropathy; Proliferative Diabetic Retinopathy and other related health conditions; minimize the need for medication and more intrusive interventions and encourage patients to adopt long term, healthy lifestyle habits.
